Author: Dr. Jay Calvert The use of prosthetic implants for breast reconstruction began in the early 60s of the last century with silicone-gel filled implants. Over the following years, breast implant technologies and surgical techniques have evolved markedly, resulting in improvement in the quality of the reconstructed breast.
Currently there are multiple surgical procedures of prosthetic breast reconstruction and numerous types of implants with variable shapes, textures, and filling materials for the plastic surgeon to choose from. The popularity of single stage implant reconstruction has declined over the years with the development of two stage expandable implant reconstructions. Early experience with breast implant tissue expanders used smooth surface, round devices with remote ports for injection of the implant’s filling. These devices were fraught with problems, including fibrosis of the capsule of the breast, poor expansion of the breast pocket and failure of the breast implant as a consequence of mechanical problems with the filling port.
The current evolutionary era of tissue expanders in breast reconstruction implants uses textured surface and anatomically shaped tissue expanders with integrated valves. These devices have a semi-rigid back to allow for preferential expansion of the implant in the front plain which aids in optimizing the overall volume of the breast. Implant design also allows for preferential expansion in the region of the lower pole of the reconstructed breast, so as to create a better match with a natural breast.
